Patients access the mobile app for free. Providers and researchers pay a subscription fee to access data that patients choose to share.

How does Forma Health reduce time per visit?
We reduce the inefficiency of patients communicating complex history by providing them both a chart and AI-driven narrative of each tracked topic through the mobile app. The physician can proceed with the right facts and without extraneous detail, all while normalizing personal and cultural biases.
What types of workflow and data access options do you support?
We make the data available through HL7 and FHIR integrations, and offer a Physician Portal to access patient and population-level data. Patients can share their AI-driven charts and narratives via printable summaries and their patient app.
Does using the Forma Health platform negatively impact your workflow?
No. The patient-reported observations can be integrated into existing workflows (EHR, patient summaries, etc.). Accessing data in the physician portal is optional and most helpful in reducing the time to understand complex patients.

How is the research-ready data structured and accessed?
We map all data collected to international vocabularies and store each observation in FHIR and an international common data model. Researchers have three options to access the data from simple previewing through our portal, direct access, or exporting to their dataset. All data access for research is de-identified and protected.
Does the dataset include medical records?
Coming soon. Because we establish a relationship with the patient, we are uniquely positioned to retrieve medical records digitally and physically that are reliably associated with the patient. We have the capability to ingest both structured and unstructured records and have pioneered a “tactical curation” process to quickly identify, prioritize, and structure unstructured data to include in analyses.
